Creating a Form
Forms let anyone fill something out and send it to your church, connect cards, prayer requests, volunteer sign-ups, RSVPs, and more. Each form gets its own public link you can share anywhere.
Before you begin
- You need the Admin or Staff role.
Step-by-step
- Go to the Forms page. In your dashboard, open Forms.
- Click New form. Give it a Name, and an optional Description (shown above the form on its public page). Click Create form to open the builder.
- Add your questions. Click Add question, choose a question type, and write the question. Repeat for each thing you want to ask. You can drag questions to reorder them.
- Mark what's required. Turn on the Required toggle for any question people must answer before they can submit.
- Choose who can submit. Under who can fill out the form, pick Anyone, Signed-in users, or Members only.
- Preview it. The phone preview beside the builder shows exactly what people will see. Use the Visitor / Member switch to check both.
- Publish. Click Publish form. Only published forms accept responses, and you'll need at least one question.
- Share the link. Copy your form's link (
churchday.co/your-church/f/your-form) and share it anywhere, in an email, a text, or on your church page.
Question types
- Short answer and Long answer: typed text.
- Dropdown, Multiple choice, and Checkboxes: pick from options you set.
- Limited slots: options with a set number of spots that fill up and lock when full (great for meal counts or serving spots). See Limited slots sign-ups.
- Yes / No, Number, and Date.
Good to know
- New people are added automatically. When someone fills out your form and includes an email, they're matched to an existing member or added to your People list, without overwriting anything already there.
- Show a question only when it's relevant. Any question can be set to appear only after someone picks a certain answer to an earlier question ("Show this question only when…").
- Answers can do more than record data. A choice answer can automatically tag people or add them to a group or team, and a question can update the person's profile (like their phone or birthday). Set these up on purpose, check each question's advanced options.
- See your responses two ways. The Submissions tab lists every response with a tally for each choice question. The Analytics tab shows visitors, submissions, and conversion. See Understanding your analytics.
- Deleting a form or a response is permanent and removes all of its submissions.
